TeleHealth is state of the art remote care technology that transmits information telephonically. Patients currently receiving services from Livingston Memorial Visiting Nurse Association are eligible for this program.

Benefits Include:

• Visual feedback through videography.
• A telephonic stethoscope that captures real time assessments of the patient's heart & lung sounds.
• Remote analysis of the patient's blood pressure, blood glucose, weight and oxygen saturation that provide useful data and statistics which are printed and sent to the attending physician on a weekly basis.
• Medication reconciliation & education targeted toward medication compliance.
• Supplemental remote monitoring in addition to scheduled clinician visits that educate, monitor and support patient self-management of chronic illnesses.
• Preventing unscheduled hospitalizations and/or emergency room visits.

Ideal candidates include patients with the following chronic illnesses:

• Cardiac Disease
• Pulmonary Disease
• Diabetes and Associated Conditions
